Joint Replacement Passport Program
The Passport Program enables you and your care team to track important milestones together during every phase of the joint replacement process, from pre-surgical education to postoperative care, rehabilitation and discharge.
Your in-depth education begins immediately after you schedule your surgery, when you and your surgeon review your current health status and risk factors. You’ll then begin meeting with our specialized care team as they help prepare you (and your home) for surgery and beyond. After surgery, you’ll receive a passport specific to your joint replacement needs. This passport will guide you through the important goals and milestones related to your care, including anti-coagulant therapy, bowel care, pain management, rehabilitation, dietary goals, and discharge preparation. Throughout your stay, your clinical team will stamp your passport as your reach your various milestones.
Once you and your physician have decided joint replacement surgery is right for you, the next step is to attend one of our free seminars to learn more about the procedures. Call 360-744-6695 for dates and times.
