Does insurance cover weight loss surgery?
Many insurance plans will help cover the costs of weight loss surgery, but coverage varies according to your medical history and individual benefits of your policy.
Contact your insurance provider to find out if weight-loss surgery is covered under your policy. Ask for a description of your benefits related to weight-loss surgery in general and if only specific procedures are covered. If your current policy does not cover weight loss surgery, contact your employer’s HR department to see if there is another policy that does.
Washington State Medicaid, Department of Social and Health Services (DSHS), CHPW, SSI, Regence Healthy Options, Molina, Provider One and Medicaid are not accepted plans at Franciscan Center for Weight Management. How quickly will my insurance company approve surgery?
Some insurers may require a record of up to six months of unsuccessful medically supervised weight loss regimen before granting approval. Other standard criteria includes:
- Proof of BMI of 40 or above, or
- Proof of BMI between 35 and 40 and documentation of associated conditions like diabetes and sleep apnea
What companies recognize Franciscan as a preferred provider?

If my insurance company does not cover weight loss surgery, what are my options?

What if I have Medicaid, DSHS or Molina?
The Franciscan Center for Weight Management refers Washington State Medicaid, including DSHS, CHPW, SSI, Regence Healthy Options, Molina and Provider One, to the Center for Bariatric Surgery at the University of Washington Medical Center.
